Nurse Practitioner or Physician Assistant – Adult Primary Care
Location: Hartford, CT area
Must have primary care experience
Salary Range: $104,312 – $167,772.80 (commensurate with experience)
 


Clinic Hours:
Monday – Thursday: 8:00 AM – 5:30 PM
Friday: 9:30 AM – 5:00 PM
Closed for lunch daily: 12:00 PM – 1:00 PM


A mission-driven, community-based outpatient clinic is seeking a Nurse Practitioner (APRN) or Physician Assistant (PA) to join their Adult Medicine team. This is a primary care role with a strong focus on preventive health, chronic disease management, and wellness for a diverse adult patient population.

Key Responsibilities:

  • Provide comprehensive adult primary care services, including diagnosis, treatment, and follow-up

  • Collaborate closely with physicians and interdisciplinary team members

  • Refer patients to specialists and coordinate care as needed

  • Maintain timely and accurate documentation using the EHR

  • Participate in quality improvement and compliance initiatives

  • Deliver culturally competent care in a diverse, urban setting

  • Perform other clinical duties as assigned


Qualifications:

  • Graduate of an accredited Nurse Practitioner or Physician Assistant program

  • Licensed or license-eligible in the State of Connecticut

  • Eligible for DEA and CT controlled substance registration

  • Strong clinical assessment, diagnostic, and communication skills

  • Comfortable working in a fast-paced outpatient setting

  • Experience in urgent care or ambulatory care (1–2 years preferred, not required)

  • Familiarity with electronic medical records and standard documentation practices
